The role you’ll contribute:
The Clinical Pharmacist is responsible for the effective implementation, integration, and coordination clinical pharmacy services and ensuring that optimal clinical pharmacy services are provided on a daily basis to patients of the designated practice and for providing certain consulting and support services to Adventhealth Polk.
The Clinical Pharmacist facilitates the development, implementation, and maintenance of systems to promote desirable patient outcomes through the use of appropriate, safe, and effective medication therapies using evidence-based standards of care.
The Clinical Pharmacist will work collaboratively with the health care team and patients to optimize medication therapy and quality, and economic outcomes. The pharmacist communicates drug information, provides comprehensive medication management, optimizes preventative care, supports seamless transitions of care through reconciling of medications, and provides education to patients and providers.
The value you’ll bring to the team:
- Maintain a model of ambulatory care based pharmacotherapy practice at The Family Health Centers.
- Lead pharmacotherapy appointments including, but not limited to, individual or co-visit clinical appointments, Medicare Annual Wellness Visits, and Chronic Care Management appointments.
- Utilize EHR and physician, nurse practitioner, physician assistant, and clinical support staff referrals to identify patients in need of comprehensive medication management
- Identify medication access and formulary issues and determine cost-effective treatment alternatives or refer patients to community resources for medication access.
- Document medication management services within the EHR such that clinical pharmacy activities may be monitored for clinical and economic impact and may be billed according to current billing practices for clinical pharmacy services.
- Provide consultation to the clinical providers to assist in the individual care of the patients. Serve as a drug information resource for health care providers, lay groups, and learners in Western North Carolina on pharmacotherapy issues.
- Address quality improvement as related to pharmacy.
- Chemo, Compounding experience is preferred
Qualifications
The expertise and experiences you'll need to succeed:
Required:
- Registered Pharmacist with an active an unencumbered license to practice in North Carolina or meets eligibility requirements for endorsement by the Board of Pharmacy
- Must be a graduate of an accredited College or School of Pharmacy ( Pharm.D preferred)
- Experience in the use of analytical and problem-solving skills required
- Ability to direct, facilitate, and collaborate effectively with a variety of work teams and health professionals.
