About This Opportunity
We are seeking a dedicated and experienced
Pharmacist to join our team. In this role, you will provide high-quality pharmaceutical care, ensuring optimal medication-related outcomes to improve patients' quality of life. The ideal candidate will take personal accountability for patient outcomes, collaborate effectively with healthcare professionals, and maintain compliance with industry standards.
Key Responsibilities
- Medication Review: Evaluate prescriber orders for safety, efficacy, and appropriateness for dispensing across multiple EMRs.
- Therapeutic Recommendations: Collaborate with physicians and prescribers to recommend optimal therapy solutions following ASHP standards.
- Drug Information: Provide critical drug-related information to healthcare providers to ensure safe medication practices.
- Patient Counseling: Educate patients on medication usage, side effects, and expected outcomes.
- Age-Specific Care: Demonstrate knowledge of and meet the needs of patients from neonates to geriatrics.
- Documentation: Accurately document all completed tasks within the order management system.
- Customer Focus: Foster excellent relationships with contracted remote facilities and healthcare teams.
Qualifications
- Education:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree required.
- Completion of an ASHP-accredited residency or hospital experience preferred.
- Licensure & Certification:
- Pharmacist license in the residing state (required).
- Must obtain four additional state licenses within 120 days of hire (specific states to be determined).
