Case Manager Pharmacy - Evening Shift - JPS Health Network
Maxor National Pharmacy Services is seeking a highly competent, compassionate and caring nationally certified pharmacy technician for a Case Manager position in the Emergency Department located within John Peter Smith Hospital. Qualified candidates must be excellent communicators who are very detail oriented and thorough, possess highly efficient time management skills, and be self-motivated. Maxor manages the pharmacies at JPS Health Network.
JPS Health Network is a $950 million, tax-supported health care system for Tarrant County in North Texas. Licensed for 578 beds, the network includes an acute care hospital and more than 40 community-based clinics, including 19 school-based clinics. John Peter Smith Hospital is home to Tarrant County’s first and only Level 1 Trauma Center, its only psychiatric emergency services site and has the largest hospital-based family medicine residency program in the nation.
Job Summary:
The Case Manager Pharmacy will support JPS pharmacy patient throughput and Pharmacy Team with coordination of safe and efficient patient prescription delivery. The Case Manager will work with multi-disciplinary teams including pharmacy, nursing, providers, case management and other roles as needed.
Responsibilities
Essential Job Functions & Accountabilities:
- Documents in the Electronic Medical Records.
- Assures timely consistent communication and coordination with patient care team and pharmacy
- Monitors prescription care plan for patients and works with multidisciplinary team to eliminate barriers.
- Solves problems relating to the patient prescriptions.
- Work as a frontline pharmacy technician to assist with patient care, if needed.
- Ability to work with minimal supervision.
- Strong ability to prioritize and stay organized.
- Cooperates in maintaining a clean, orderly, safe, and professional environment.
- Ensures medication management processes meet all legal and regulatory requirements and assists with monthly reporting.
- Participates in performance improvement (PI) activities which improves patient care, specifically best in class pharmacy processes.
Inpatient Patient Care Coordinator:
- Interviews patient and completes comprehensive Medication History.
- Records known allergies.
- Completes penicillin allergy screening tool with patient to assist with de-labeling allergy.
- Acts upon Patient Care Coordinator Consults entered by nursing and providers.
- Eliminates medication barriers during care transitions and improves medication access at hospital admission and discharge.
Outpatient Patient Care Coordinator:

- Monitor patient prescription orders and ensure proper prioritization within pharmacy operations.
- Verified patient demographic information and educates patients on delivery option.
- Tracks prescription orders via pharmacy tracking program to ensure correct delivery information.
- Monitors patient’s prescription delivery to ensure prescription medications are delivered or picked up timely.
- Provides technical and operational support of drug distribution services for all outpatients.
- Collaborate with state programs to help ensure timely medication delivery.
- Assist in identifying cost saving alternatives for patients through the utilization of manufacture coupons or patient assistance programs
Qualifications
Qualifications:
Required Education and Experience:
- High School Diploma or GED equivalent.
- 3 plus years of relevant work experience.
- Experience supporting team members in the development of their skills and abilities.
- Current registration through the Texas State Board of Pharmacy.
- National certification through National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB).
- Medication History Certification within 6 months of hire (inpatient patient care coordinator only.
Preferred Qualifications:
- Bachelor's degree in business or health related field.
Knowledge, Skills & Abilities:
- Knowledge of third party insurances and relationship to operations and the other JPS facilities
- Knowledge of JPS computer systems• Knowledge of third party coverage
- Skill in communicating with patients
- Skill in basic math required to interpret Physicians instruction(s)
- Ability to interact with patients/visitors/staff of diverse cultural and socio-economic backgrounds displaying outstanding customer service
- Ability to prioritize work and handle stressful situations
- Ability to work cooperatively and independently as law permits requiring minimum supervision with the willingness to work outside of assigned area of responsibility
- Ability to adapt mentally to perform daily activities
- Ability to grasp new ideas and concepts quickly in order to follow directions concerning new policies and procedures with handling prescriptions.
- Ability to read and understand basic medical terminology as used by physicians on prescriptions
- Ability to assess situations and suggest alternative solutions to meet the needs of patients
- Ability to communicate effectively verbally and in writing
Required Licensure/Certification/Specialized Training:
- Current pharmacy technician registration with the Texas State Board of Pharmacy.
- National certification recognized by the Texas State Board of Pharmacy. National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB).
