The
Patient Outreach Coordinator is responsible for outreaching to and engaging with patients that are due for medication refills and key preventive health screenings. Reporting to the Vice President of Clinical Performance, the Patient Outreach Coordinator supports quality programs by communicating clinical care gaps to patients and supporting patients in their journey to get timely care. The Patient Outreach Coordinator will help patients to close care gaps by making appointments, liaising with providers and pharmacies, and coordinating care, as needed, in the home. The Patient Outreach Coordinator also updates patient charts and supports the review, abstraction, and submission of quality measure information to improve quality performance. As an integral part of the Quality Team, the Patient Outreach Coordinator has a commitment to service excellence and continual learning.
This position is full-time, Monday - Friday. Employees are required to work during our normal business hours of 8:00am - 5:00pm EST. It may be necessary, given the business need, to work occasional overtime.
We offer weeks of on-the-job training. The hours of training will be based on your schedule or will be discussed on your first day of employment.

Primary Responsibilities
- Works under the direction of the Vice President of Clinical Performance and provides outreach to and engagement with patients who are due for medication refills and key preventive health screenings
- Provides exceptional customer service and supports patients in their journey to close clinical care gaps by making appointments, liaising with providers and pharmacies, and coordinating care, as needed, in the home
- Supports quality submissions for payer quality programs including those administered by The Centers for Medicare and Medicaid Services (CMS), Medicare Advantage (MA), Medicaid, and Commercial payers
- Supports activities including chart review, abstraction, and submission to payer portals
- Participates in data validation of quality reports to support patient outreach and care gap closure
- Attends and participates in quality team meetings
- Assists in the resolution of issues and elimination of roadblocks to ensure timely completion of tasks
- Performs other duties as assigned

Required Qualifications
- High School Diploma / GED
- Must be 18 years of age OR older
- Exceptional customer service and telephone skills
- Interpersonal skills and an understanding and commitment to delivery of patient-centered medical care with a team-based approach
- Organizational skills and a demonstrated ability to maintain accurate notes and records
- Ability to manage multiple and simultaneous responsibilities
- Ability to work independently, exercise creativity, and is detail-oriented
- Proficiency with Microsoft Outlook, Microsoft Excel, and Microsoft Power Point
- Ability to work Monday - Friday, during our normal business hours of 8:00am - 5:00pm EST
Preferred Qualifications
- 3+ years of experience in clinical or community health settings
- Previous experience working with patients

- Ability to keep all company sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
