Job Title: Ambulatory Care Clinical Pharmacist

Job Responsibilities:
• Provide comprehensive medication management services
• Collaborate with healthcare providers to optimize patient medication regimens
• Conduct medication therapy reviews and reconciliations
• Develop and implement patient-specific care plans
• Educate patients on proper medication use, potential side effects, and drug interactions
• Monitor and assess patient response to medication therapy
• Identify and resolve drug-related problems
• Participate in interdisciplinary team meetings to discuss patient care
• Provide drug information and clinical recommendations to healthcare providers
• Assist in the development and implementation of clinical protocols and guidelines
• Contribute to quality improvement initiatives and medication safety programs
• Document patient encounters and interventions in electronic health records
• Precept pharmacy students and residents, as applicable
• Stay current with new medications, treatment guidelines, and best practices in ambulatory care pharmacy
• Participate in ongoing professional development and continuing education activities
• Assist in managing inventory and ensuring medication safety in the outpatient pharmacy
• Collaborate with insurance companies to resolve prior authorization issues and ensure patient access to medications
• Conduct medication use evaluations and contribute to formulary management
• Participate in patient education classes and support groups, as needed
Skills Required:
• Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy
• Active pharmacist license in Texas
• Board Certification in Ambulatory Care Pharmacy (BCACP) preferred
• Minimum of 2 years of experience in ambulatory care or related clinical pharmacy setting
• Strong knowledge of pharmacotherapy, disease state management, and evidence-based medicine
• Excellent interpersonal and communication skills
• Ability to work effectively in a multidisciplinary team environment
• Proficiency in electronic health record systems and medication-related software
• Strong analytical and problem-solving skills
• Commitment to patient-centered care and continuous quality improvement
• Ability to manage multiple priorities and adapt to changing circumstances
• Excellent time management and organizational skills
• Familiarity with medication safety principles and practices
• Knowledge of regulatory requirements and healthcare accreditation standards
• Experience with population health management and chronic disease state programs
• Proficiency in Microsoft Office suite and database management
• Ability to precept pharmacy students and residents
• Strong presentation and teaching skills
• Commitment to ongoing professional development and staying current with pharmacy practices
Job Details:
Area: El Paso, TX
Licensure Requirements: Must have open, active, and in good standing TX State Pharmacist License
Education: Pharm.D. or B.S.
Years of Experience: 2 years
Schedule: Monday – Friday; 8 Hour Day Shift
Background/ Drug Requirements: Must be willing to submit to a background check, health and drug screen.
