The Staff Pharmacist fills orders, monitors patient drug therapies and provides drug information. Staff Pharmacists supervise and direct supportive personnel. In the absence of the Director of Pharmacy, a Staff Pharmacist may be required to assume the essential responsibilities and perform the duties of the Director of Pharmacy.

1. Interprets drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles.
2. Compounds and dispenses pharmaceuticals including sterile products and parenteral nutrition products accurately.
3. Issues controlled substances to patient care areas and maintains records as required by law.
4. Adheres to hospital and departmental policies and procedures. Complies with all requirements related to risk management, safety, security, fire, and infection control. Complies with all applicable federal and state laws, rules and regulations.
5. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
6. Assists with pharmacokinetics consult service and drug dosing per hospital protocol. Provides clinical clarifications, cost effective therapeutic alternatives, accurate drug information and education to practitioners and patients as needed.
7. Review/interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed.
8. Reads, extracts and interprets information in patient charts accurately.
9. Detects and reports suspected adverse drug reactions accurately and in timely manner.
10. Sustains the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.
11. Documents all clinical activities and interventions accurately and completely.
12. Participates in the quality improvement and medical use review activities of the department.
13. Is involved in the development and presentation of orientation, education training programs for the medical staff, nursing services and other hospital staff.
14. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy support personnel.
15. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
16. Completes all assigned medication storage area inspections in a timely manner. Identifies and replaces outdated and unusable drugs.
17. Keeps pharmacy areas and equipment clean, neat and well organized.
18. Oversees operation of the pharmacy in the absence of the Director of Pharmacy as necessary.
19. Other duties as assigned.
Qualifications Licensed as a pharmacist in the State of South Carolina. Hospital experience preferred. Education: South Carolina Pharmacist License Experience: Requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
